PASSPORT APPLICATION AUTHORISATION FORM (print only)

I hereby authorise The British Blue Cattle Society, in conjunction with Holstein UK & Ireland, to make passport applications to BCMS on my behalf, subject to the following conditions:

  • I have read the DEFRA ‘Notes for Guidance’ about cattle passports.

  • I understand the cattle passports may be supplied on the basis of the information submitted to Holstein UK & Ireland. I understand that any cattle passport issued for a domestic male bovine as a result of any passport applications will also serve as subsidy documents (CID/CCD) in relation to the Beef Special Premium Scheme (BSPS).

  • I will not make any duplicate applications for cattle passports or CID/CCD documents.

  • The information submitted to Holstein UK & Ireland will be correct to the best of my knowledge.

  • I agree that any Inspector or Veterinary Inspector appointed by the appropriate Minister or local authority, accompanied by any person he considers necessary, may enter my holding or holdings to check the accuracy of details submitted to Holstein UK & Ireland. I will give all reasonable assistance to such persons.

I authorise The British Blue Cattle Society to debit my account with such fees and charges as may be payable.
